→ Intended Applicants: This form is only for those listed as an exhibitor (or co-exhibitor) of  SPIE exhibitions. 

→ Application Guidelines: 

• You must be listed as an exhibitor (or co-exhibitor) of this exhibition or your request will not be processed
• The name must be spelled to match the passport information exactly; any spelling differences may cause visa delays and/or denials.
• Once you click "Mark as Complete", you must also click "Submit", otherwise your application will NOT be processed.
• Requests are processed in the order received.

Note: SPIE is unable to contact embassies or consulates on behalf of or in support of any individual making arrangements to attend an SPIE event.

→ After Submitting:

• You will receive an email from noreply@mail.smapply.net with a link to return to your application and download your official invitation letter(s) as a PDF file(s). Please check your spam/junk folder if you do not see the email. 
  • Note: To ensure delivery of SurveyMonkey Apply emails, ask your organization to allow the email addresses listed here—this webpage also includes details you can provide your IT department for troubleshooting.
• Sign back into your application to download your letter, then follow the instructions to approve or request changes to your letter.
