Intended Applicants: This form is only for participating Authors/Speakers, Chairs, Committee Members, and qualifying attendees of upcoming SPIE meetings in Asia. 

Application Guidelines:

• Please complete one form per person. Include requests for accompanying family members in the same application. 
• Requests are processed in the order received.

You will receive an email with a link to download a PDF. SPIE is unable to contact embassies or consulates on behalf of or in support of any individual making arrangements to attend an SPIE event.
